just include a photo, or detailed description of yourself (the more details the better! things like how you wear your hair and such. .. ), and any accessories you want included.
wearing any of the following:

• hogwarts uniform (with or without robe overtop)
  
• Quidditch uniform (again, wiht or without robe overtop, AND specify position for accessories, i.e. snitch/bludger. . .)
  
• street clothes (please describe things you normally wear)
